Criminal Offense Declaration Form for Volunteers

SPFHA has provided a Criminal Offense Declaration Form to be submitted annually for volunteers who have already submitted a Police Clearance Check (PRC).

All of the Volunteers who have provided a PRC (Police record Check) to the Association within the last 3 years, and who intend to reapply as a Volunteer with us again this upcoming season, will now be able to print off a COD (Criminal Offense Declaration ) form and continue Volunteering without having to bring in a new PRC. This form needs to be completely filled in, and signed, before submitting it to a League Representative. Be sure to place the confidential form into a sealed envelope, and write your name on the outside of it, before submitting it to a League Representative either in person, or by way of our mail box, which is located at Forest Glade Arena. The physical mail slot is located on the SPFHA office door at the far North end of the building near the far rink.
